Sometimes you are ready to leave the hospital but need additional care before you can return home safely. When you need transitional care, you can depend on Adventist Health Tillamook to provide the care you need right in your own hometown.

You can get the short-term orthopedic, neurological and medical rehabilitation you need without having to be far away. Serving the central and north Oregon Coast, our skilled team of licensed and certified medical professionals actively work with you to meet your health goals and safely return home.

And since you're close to home, you may be able to leave the medical center to visit with family, friends and pets, depending on your care program. You can enjoy your meals in your room or get a change of scenery in our on-site café.

A team dedicated to getting you home

Adventist Health Tillamook's transitional care swing bed program offers an entire team to help you through each step of your healing journey. Your team includes care managers, hospitalists, physical therapists, occupational therapists, speech therapists, registered nurses and certified nursing assistants.

Your highly engaged care team provides customized services including:

• Activity coordination
• Care management and discharge planning
• Chaplain consultations
• Nutritional services
• On-site physicians, X-ray and lab
• Physical, speech and respiratory therapy
• Pharmacist consultations and IV medication
• Wound care

Medicare and most private insurance companies will pay for transitional care for a limited time. Medicare patients receive 20 days of skilled nursing care without a copay. Copays may apply after 20 days.
