CDL Physicals
At Adventist Health Castle Urgent Care, getting you on the road is our goal.
We offer CDL physicals, appointments are recommended but walk-ins are accepted.
We encourage you to schedule your appointment at least 4-weeks before the expiration date of your current certificate. Call (808) 456-2273 to schedule an appointment.

Schedule Your Appointment
Appointments can be scheduled at any of our four locations.
We recommend arriving 30 minutes prior to your appointment to complete any additional paperwork.
Note: We may require additional medical clearance(s) from your primary care physician, specialist and/or may require you to return for a follow-up appointment before you obtain your certificate.
