Weight Management Programs

The Adventist Health Employee Health Plan covers the cost of the following weight management programs at 100 percent:

  • Livongo weight management
    Livongo’s weight management program connects you with a certified health coach who helps make living healthy and tackling your weight challenges easier. It provides you with a connected scale to automatically track your progress.

  • Weight Watchers
    Weight Watchers focuses on healthy eating and increasing activity by using a ‘smart points’ plan based on calories, saturated fat, sugar and protein. Weekly group classes are available at local meeting sites and some Adventist Health facilities. 100% of program fee covered upon proof of 80% attendance.

    Physician’s referral required. Lifetime maximum of 12 months. Learn more at www.weightwatchers.com.
  • CHIP (Complete Health Improvement Program)
    CHIP is a lifestyle enrichment program designed to reduce disease risk factors through the adoption of better health habits and appropriate lifestyle modifications. Participants will complete a health risk assessment and blood draw. Program may be completed online.

    100% of program fee covered upon proof of 80% attendance. Physician’s referral required. Learn more at www.chiphealth.com.

How does it work?

For reimbursement from Adventist Health for Weight Watchers or CHIP, ask your doctor to sign the referral form before the program begins. Sign up for the program and attend all required sessions. You must file for reimbursement and must attend 80 percent of the sessions to be reimbursed. For Weight Watchers only, you will need to record your attendance on the attendance form.

Completed Weight Watchers or CHIP? Here’s how to file for reimbursement:

  • Ask for a completion certificate from the program.
  • Fill out the reimbursement form. Forms are available on Connect.
  • Fax the referral form, completion certificate (or attendance form) and receipts to: (916) 781-2441
