COVID-19 update: We have stringent measures in place to keep our patients and staff safe, with highly trained infection prevention practitioners closely following the guidelines from the CDC and California Department of Public Health. A few of the safety precautions you can expect include symptom and temperature screening prior to entry, universal masking, enhanced facility cleansing, reconfigurations to allow social distancing and visitor restrictions. Resident Staff Update 01/16/24: The information below is updated regularly to help ensure residents, family, and the community are kept up to date on The Fountains Skilled Nursing current COVID-19 status. In the event of a positive case, we have a mitigation plan in place to protect our healthy residents and team members. This includes immediately isolating residents with suspected or confirmed COVID-19. In addition, team members with suspected or confirmed COVID-19 are directed to stay at home until they are fully recovered. We are currently testing all of our staff twice a week for COVID.
- There is currently 1 confirmed, COVID-19 positive residents in our COVID-19 isolation unit.
- There is currently 0 team members who are unavailable to work because of a COVID-19 positive test.
- Since January 1, 2020, there have been 205 residents* and 44 team members who have tested positive for COVID-19
The Fountains is a 145-bed skilled nursing facility offering around-the-clock long-term nursing care and short-term restorative therapies. A new era in quality physical rehabilitation
Physical rehabilitation patients will recover in spacious semi-private rooms with excellent personalized care from our experienced rehabilitation team comprised of physical, speech and occupational therapists.
The center is designed to help patients maintain and improve their independence. We offer rehabilitation for:

- Strokes
- General Surgeries
- Pulmonary and Cardiac
- Back and Neck pain
- Multi-trauma
- Wounds
Our team is dedicated to each patient as they strive to overcome limitations and maximize their potential in a spacious state-of-the-art gym as well as an outside mobility course where patients can exercise at stations and on varied surfaces while enjoying the outdoors.
