Social Work & Case Management

Experienced social workers and case managers are available to patients of all ages at Adventist Health Sonora. Our social services department has full-time, master's-trained social workers prepared to help patients and families manage the medical, emotional and social issues related to their illness or injuries. Adventist Health Sonora’s full-time case managers are specially trained registered nurses who provide information and support for patients and their families.

To help in coordinating medical care for individual patients, case managers and social workers assess the patient's physical and emotional needs and arrange for services to meet those needs. They also help develop discharge plans. They may also contact patients and their families after discharge to evaluate continuing care needs.
