Medical Records
Patients have the right to request a copy of their medical records. In order to obtain your medical records you must submit a written request and proof of identification. There are two ways to request these records:

Authorization Release Medical Information - Spanish - Come to the HIM (Health Information Management) office located in Hanford, Selma, Reedley, Tulare or the Adventist Health Medical Office (clinic) that you need records from. Please see location and hours of operation listed below.
Adventist Health Medical Centers
Requests must be submitted to the Health Information Management Department (Medical Records). The department hours are as follows:
Hanford
Monday - Thursday, 8:00 a.m. to 4:30 p.m. Open during lunch.
Friday, 7:30 a.m. to 4:00 p.m. Open during lunch.
Selma
Monday - Thursday, 8:00 a.m. to 4:30 p.m. Closed for lunch – 12:00
p.m. to 1:00 p.m.
Friday, 7:30 a.m. to 4:00 p.m. Closed for lunch – 12:00 p.m. to 1:00 p.m.
Reedley
Tuesday and Thursday, 8:00 a.m. to 4:30 p.m.
Closed for lunch – 11:30 a.m. to 12:00 p.m.
Tulare
Monday - Friday, 8:00 a.m. to 4:30 p.m. Open during lunch.

- Personal record copies are provided free of charge with a valid authorization.
- Records can be mailed or picked up at Health Information Management.
- Records may also be requested to be provided in electronic format (CD).
Adventist Health Medical Offices
Requests must be submitted to the clinic where services occurred. Please contact the appropriate clinic for hours of operation.
NOTE: Records are sent to your doctor at no charge.
