Adventist Health Glendale Earns 23rd Consecutive “A” Grade for Hospital Safety
Only Los Angeles County hospital with 23 straight “A” grades from The Leapfrog Group
Adventist Health Glendale has earned its 23rd consecutive “A” Hospital Safety Grade from The Leapfrog Group, an independent national nonprofit watchdog focused on patient safety.
The latest grade designates Adventist Health Glendale as a “Straight A” hospital, reflecting sustained excellence across more than a decade of grading cycles. Adventist Health Glendale has maintained an “A” in every grading cycle since 2015, underscoring an organization-wide commitment to safety, reliability, and high-quality care.
With this spring 2026 rating, Adventist Health Glendale leads Los Angeles County with 23 consecutive “A” grades for hospital safety. It is also the only hospital in California to achieve this specific milestone and one of only two hospitals nationwide with exactly 23 consecutive “A” grades, according to The Leapfrog Group.

Sustaining this level of performance for 23 consecutive grading cycles speaks to our deeply rooted culture of safety and to the daily commitment of our physicians, nurses, and clinical teams who prioritize patient safety in every interaction.”
The Leapfrog Group assigns an “A” through “F” grade to general hospitals across the country using more than 30 evidence-based measures of patient safety. The grades evaluate errors, accidents, injuries, and infections, as well as the systems hospitals have in place to prevent harm.
Adventist Health Glendale’s 23 consecutive “A” grades represent a sustained record of excellence since the hospital began reporting for Leapfrog grading consideration. Its continued performance places the hospital among an elite group of consistently high-performing hospitals nationwide. In the spring 2026 cycle, 284 hospitals were graded in California, and nearly 3,000 hospitals were graded across the United States.

The Leapfrog Hospital Safety Grade is the only hospital ratings program in the United States focused exclusively on preventable medical errors, injuries, and infections that can lead to harm or even death. The grading system is peer-reviewed, fully transparent, and free to the public. Grades are updated twice annually, in the fall and spring.
