Adventist Health Glendale Achieves 22nd Consecutive “A” for Hospital Safety, Earning Elite National Status

Adventist Health Glendale is celebrating a record-setting milestone in patient safety. For Fall 2025, the hospital has once again earned an “A” Hospital Safety Grade from The Leapfrog Group, an independent national nonprofit watchdog that evaluates hospitals on how well they protect patients from preventable harm.

This latest rating marks Adventist Health Glendale’s 22nd consecutive “A” — a distinction achieved by only a select few hospitals across the nation. According to The Leapfrog Group , only 11 U.S. hospitals have earned an “A” during every grading round since 2012. Adventist Health Glendale began reporting in 2014 for 2015 grading consideration.

A decade of straight “A”s

Leapfrog’s Hospital Safety Grade program evaluates hospitals twice a year, in the spring and fall, using more than 30 measures focused on:

  • Preventing infections
  • Reducing errors and complications
  • Protecting patients from falls and other injuries
  • Ensuring strong safety systems and communication practices

Hospitals receive a letter grade from A to F based on their performance. While about 32% of hospitals nationwide earned an “A” for Fall 2025, only a very small group has maintained top marks every single grading cycle for a decade.

“Earning 22 straight ‘A’ grades is an extraordinary accomplishment and a direct reflection of our teams’ unwavering commitment to safety, quality, and reliability,” said Alice Issai, president of the Adventist Health Glendale and Simi Valley Service Area. I am incredibly proud of our team’s collective dedication and of what this achievement means for the communities we serve.”

What the Leapfrog grade means for patients

The Leapfrog Hospital Safety Grade is different from many other hospital ratings because it looks exclusively at safety— specifically, whether hospitals have strong practices in place to prevent harm before it happens.

Examples of the types of measures included are:

  • Hospital-acquired infections, such as bloodstream or urinary tract infections
  • Medication safety and barcoding practices
  • Safe surgery protocols, including checklists and time-outs
  • Nursing staffing and responsiveness
  • Communication about medications and discharge instructions

“My sincere congratulations to Adventist Health Glendale for upholding patient safety standards through thick and thin, achieving an ‘A’ Hospital Safety Grade for 22 consecutive rounds of the Safety Grade,” said Leah Binder, president and CEO of The Leapfrog Group. “Consistently earning an A demonstrates a long-term, organization-wide commitment to putting patient safety first.”

A commitment to our community

Adventist Health Glendale has held an “A” in every Leapfrog grading cycle since 2015, reflecting a sustained, organization-wide focus on safety, quality, and continuous improvement.

Only one other nearby hospital received an “A” rating for Fall 2025. In the Spring 2025 cycle, no other hospitals in the areas of Glendale, La Cañada, Burbank, Pasadena, or Eagle Rock earned an “A” rating.

“For our community, this recognition is more than a badge or a score,” Issai said. “It’s a promise. Whether it’s an emergency, surgery, cardiac care, or any hospital stay — patients can trust that Adventist Health Glendale is intensely focused on keeping them safe.”
