Enrolling in our care

To avoid delays in you and your family’s care, please complete the following steps before you schedule an appointment: 

  1. Choose your primary care doctor: scroll down to “Our Providers” to select from our trusted providers. 
  2. Update your health plan: call the member services number on the back of your insurance card, and ask to change your primary care provider to one of our trusted providers. 
  3. Watch for your new insurance: your updated card will arrive in 30–45 days. 
  4. Schedule your first visit: once your plan is updated, call us at 747-212-3441 to book your appointment. 

Please note: We cannot see you and your family unless your insurance is assigned to one of our providers, so be sure to make the switch today.
