Transitional Care Swing Bed Program

Adventist Health Reedley offers community members a unique recovery experience with the Swing Bed program. This department is located inside of the Adventist Health Reedley hospital medical surgical wing and an offers a great transition to post-acute care for patients prior to returning home.

This program can be considered when patients may need:

  • Rehabilitation services (PT/OT/ST) for stroke, joint replacement, ortho surgery, amputations, GI, Debility, and TBI in addition to other diagnoses requiring therapy
  • Administration of IV medications and fluids
  • Complex wound care. decubitis care and sterile dressing changes
  • Tube feedings and nutritional stabilization
  • Daily respiratory therapy services
  • Additional skilled services

Residency in the Swing Bed Program is covered by Medicare as well as private insurance plans.

Medicare patients must meet the following criteria:

  • Three-day qualifying stay as an acute inpatient within the past 30 days
  • Physician referral for necessary daily skilled services related to hospitalization
  • Resident has Medicare Part A and benefit days available
