At Adventist Health While Memorial Montebello, we work to ensure our entire community has the support it needs to access care. That’s why our Community Well-being Center is here, offering you helpful services like health insurance enrollment, health and wellness workshops, and much more.
Health, wholeness, and hope — right in your neighborhood
The Community Well-being Center (CWC) is your one-stop hub for wellness, resources, and connection. Our services are designed to help individuals and families live their healthiest, happiest lives — all right here in Montebello and surrounding communities. Services include:
Free programs and services for all ages
Our Community Well-being Center offers our Montebello community a variety of free services that can help you with your health and wellness, transportation, immigration, and more. Once you schedule an appointment, simply walk into our Community Well-being Center and someone from our bilingual staff will be happy to show you what we offer and get you started.
Mexican American Opportunity Foundation (MAOF)
In collaboration with MAOF, our Community Center offers special assistance with the following:
- Taxi vouchers and bus tokens
- Assistance with completion and translation of the following forms: Social Security, Medicare, home energy applications, more
- Referrals for the following services: Immigration, meal sites, senior housing, social/recreational activities, Social Security, home delivered meals, legal services, Home Secure Program, Handy Worker Program, and transportation needs.
¡Vive Bien! Wellness Program
The ¡Vive Bien! Wellness Program is a free program to enhance your health and quality of life. The program offers free classes and activities including:
- Workshops with doctors, pharmacists, and health experts Information and tips on preventing and managing disease
- Exercise classes
- Nutrition classes and healthy cooking demonstrations
- Social activities and events
- Information and assistance with healthcare coverage and enrollment
Health insurance and enrollment assistance
Healthcare enrollment can be confusing, so we're happy to help. We offer information, assistance, and referral services for you and your family. Our friendly bilingual counselors can take you step-by-step through the enrollment process, making sure you receive the health care coverage you need. Coverage assistance includes:
- Covered California
- Medicare
- Medi-Cal
Make an appointment and remember to bring in the following required documents for enrollment:
- U.S. birth or naturalization certificate(s)
- Three current paycheck stubs
- Social Security card(s)
- Verification of pregnancy (if pregnant)
Social services referrals
Healthcare Navigators connect you to transportation assistance, housing resources, mental health and substance use support, long-term food programs, and guidance finding trusted specialty healthcare providers.
Food access and education
Prema Food Pantry is available to anyone facing food insecurity.
Additionally, food vouchers to our weekly Farmers’ Market, access to our community garden, and sustainable farming education opportunities are available to CWC members.

Meet Araceli Marquez
Araceli Marquez, our Community Well-being Center Coordinator, is always learning from the community surrounding the Adventist Health White Memorial hospital. In fact, her career began while she was still attending Cal State LA as an undergraduate studying public health. As our Center Coordinator and having obtained her Master's in Public Health, Araceli brings valuable and innovative ideas to our programs while coordinating how we serve our community members.
