Appointment preparation 

A parent or guardian must accompany the child to their appointment. If you are unable to attend your child’s appointment, you must provide a signed note that indicates the designated adult bringing your child is authorized by you to make medical decisions for the health of your child at that appointment. A separate signed note will be required for each such instance.
