At Adventist Health Glendale, our goal is to help make the transfer of patients from referring facilities as seamless and timely as possible. The Adventist Health Transfer Center is your single source for acute transfers and direct admissions.

Expert nurses handle everything

The Adventist Health Transfer Center is staffed by registered nurses trained in critical care and emergency medicine. When you call, one of our experienced nurses will:

• Identify the appropriate hospital and specialists for your patient based on capability, capacity and patient preference.
• Facilitate a recorded call between the referring and accepting provider.
• Complete a quick registration.
• Arrange the bed assignment.
• Collect necessary clinical details for hospital admission.
• Provide admission information to the referring provider.
• Arrange for transportation, if needed.
• Answer any questions throughout the process.
• Facilitate physician to physician consults.

The Adventist Health Transfer Center is available 24 hours a day, seven days a week, 365 days a year. We are here for physicians, nurses and other authorized healthcare providers who need to:

• Transfer a patient into or out of an Adventist Health hospital or medical center.
• Admit a patient directly from a physician’s office, urgent care center or other healthcare facility.

Information to have on hand

When you call, we will ask for patient and clinical information. Please have the following information available when you call:

• Your name and call back number.
• Referring provider’s name and call back number.
• Patient details: Name, DOB, SSN, diagnosis and other relevant clinical information.
• Specialty requested
• Level of care.
• Mode of transportation (acute sites only).

*Pediatric patients can be transferred to a facility that has the capability to care for them.

Help admit a patient to a non-affiliated facility if the appropriate Adventist Health site isn’t available or doesn’t offer the necessary services.
