Viewing your health information

You have a direct link to access your own personal electronic health records immediately through hospital care and outpatient care patient portals. Log in to the appropriate patient portal to view items such as:

  • Pathology, radiology and lab results
  • Health records such as immunizations, medications and allergies
  • Hospital records such as discharge summaries and notes about procedures, progress and consultations
  • Health records to easily share with your doctors

Patient Portal and Log In

Please note that not all medical records are viewable and/or retrievable through the patient portal. For records that are not available through the patient portal, you may submit a request for a physical copy of your medical records to the site where you received care. 

To request the records, please download an Authorization for Release of Health Information below. This is required for any use or disclosure of your health information.

The patient must sign the authorization unless the patient is a minor or declared physically or mentally incompetent. If the patient is a minor, one of the patient’s parents would sign the authorization. If the patient is mentally or physically incompetent the patient’s legal guardian would sign the authorization and provide a copy of the Durable Power of Attorney of Health Care. If requesting records on a deceased patient, please see requirements on the Authorization for Release of Health Information. Please note: unsigned or incomplete requests cannot be processed.

Mail or fax the completed form to the Health Information Management office. You may also complete the Authorization for Release of Health Information form in person at the appropriate office during business hours. Hours of operation are Monday through Friday, 8:30 a.m. to 4:30 p.m. The offices are closed weekends and holidays.

Radiology Imaging

To obtain a copy of your radiology images on a CD in addition to requesting printed medical documents, please check the box “Other” on the appropriate authorization form and write “include imaging on CD” in the space. The health information management team will process your record request and submit your CD request for processing as well.

To only obtain a copy of your images on a CD with no other printed medical documents, please print and complete the appropriate Authorization for Release of Health Information form, checking the box “Other” and writing “Imaging on CD” in the space. Please contact the appropriate radiology department Monday through Friday, 7:30 a.m. to 4 p.m. for processing.

Timing and Fees

Requests are typically fulfilled within 14 business days. There are no fees associated with requests sent directly to another medical provider. Fees may be associated with requests that are sent to you or to someone who is not a medical provider. If there are any fees associated with your request, you will be invoiced by our business partner, Ciox Health. All fees are in accordance with state and federal laws.

In-Person

You may pick up a copy of your medical record in person during Health Information Management business hours, Monday through Friday, 8:30 a.m. to 4:30 p.m. The offices are closed weekends and holidays. Please indicated on the Authorization for Release of Health Information form that you would like to pick up your records in person. For your protection, we require picture identification before releasing copies of your medical records.
