What To Do If You Have A Concern

If you have a question or concern regarding the care you receive or the services rendered, please contact your nurse or physician. For non-medical issues, contact the appropriate department manager. If the concern is not resolved to your satisfaction, you may file a formal complaint by calling the Customer Loyalty Department at (323) 268-5000 x4367. If your complaint is not handled to your satisfaction, you have the right to send a written complaint to:

If a patient or family member has concerns about patient care and safety at Adventist Health White Memorial (AHWM) that cannot be resolved through the organizational leaders, then the AHWM leadership encourages the patient/family and/or others to contact the Joint Commission directly:
