For Patients of Adventist Health Physicians Network on Railroad Ave. in St. Helena

On May 11, 2019, the Adventist Health Physicians Network medical office located at 1530 Railroad Ave. in St. Helena discovered documents containing protected health information that had been inadvertently discarded in a dumpster located behind the facility. The documents may have included name, date of birth, medical account number, medical record number, visit date, diagnosis, social security number, and in some cases, patient visit summaries. To the best of our knowledge, not all records contained all the above data points. All potentially-affected patients have been notified by mail.

Even though there is no evidence that information has been used inappropriately, there are additional steps that potentially-affected patients can take:

  1. Order copies of your credit reports from any one of the credit reporting agencies for free. To order, call 877-322-8228 or go to the Annual Credit Report website at
  2. Place a “fraud alert” on your credit, which lets creditors know to contact you before opening new credit accounts. To do so, contact one of the three credit reporting agencies. (Equifax: 888-397-3742, Experian: 800-525-6285 and TransUnion: 800-680-7289)

In addition, Adventist Health Physicians Network will pay for potentially-affected patients’ enrollment in theft protection services with ID Experts®, a data breach and recovery services expert, to provide you with MyIDCare™. MyIDCare services include 12-months of credit and cyberscan monitoring, a $1,000,000 insurance reimbursement policy and fully-managed theft recovery services.
