Request Medical Records

  • A patient, or legal representative, may request a copy of medical records by downloading the Authorization to Release Medical Information form here. Please specify which portion of your medical record you need (clinic, procedure, date of service, years, etc.) and fax, mail, or drop off the completed form to the Health Information Management Department.
  • Please allow 15 days for us to prepare and release your medical records.
  • If you have an emergency situation please call us or specify on your request when it is needed and every attempt will be made to expedite your request.
  • Medical records for personal use can be mailed or picked up at our office on the ground floor of the hospital.
  • Any person picking up records needs to provide picture ID.

If you have an appointment with a provider and want your doctor to have pertinent records then they can requests records on your behalf.
