Getting Image Copies

If you need a copy of your X-rays, MRI scan, CT scan or other images for a medical appointment or other purposes, Adventist Health Simi Valley is here to help. Just follow the steps outlined below.

Step 1: Call Ahead

Call ahead to (805) 955-6360 and request for your images to be printed so when you arrive, they are ready.

Step 2: Identification

Make sure you have the proper identification before you arrive so there won’t be a delay in processing your request:

• If you’re requesting your own images, please bring a photo ID with you, such as a valid driver’s license or passport. You must be at least 18 years old to request your images.
• If you’re requesting images for a patient under age 18 and you are the parent or guardian, please bring your valid ID with you.
• If you’re requesting images for someone for whom you have power of attorney, you must bring that document with you, unless you know for certain Adventist Health Simi Valley already has the document on file. Please also bring your photo ID with you.
• If you’re requesting images for someone whose injuries prevent them from making the request in person, you must have a note from the patient that includes the patient’s name, your name and a statement specifying that the patient has designated you to retrieve the images. You must also bring your photo ID and a copy of the patient’s photo ID.

Step 3: Get your Images

You can request your images at Adventist Health Simi Valley Hospital. It is not necessary to go to the same center where you originally had the images taken. You will be asked to complete a release of records form in order to receive your images. It can take upwards of about 10 minutes to create a CD with your images, if they are not already printed. There is no charge for this service.

• Adventist Health Simi Valley, located at 2975 North Sycamore Drive in Simi Valley. This location is open from 6:30 a.m. to 6 p.m. Monday through Friday. Park in the lot off of Jones Way behind the emergency room and enter through the ER entrance. Tell the security guard on duty you need to go to the Outpatient Center, and they will direct you.
• The Aspen Outpatient Center, located at 2750 Sycamore Drive in Simi Valley. This location is open from 8 a.m. to 5 p.m., Monday through Friday.
• Wood Ranch Imaging Center, located at 124 Macaw Lane in Simi Valley. This location is open from 8 a.m. to noon and from 1 p.m. to 5 p.m., Monday through Friday.
