Employees in many positions may select a benefit level that meets their personal situation and needs. Some are fully benefited. Eligible employees may opt out of medical and dental plan benefits and receive $100 per month while enjoying all other benefits. Other eligible employees may opt out of all benefits and receive 12% higher salary than the fully benefited level. Contact the Human Resources office for more information on your benefit options.

Medical Plan

A PPO Medical Plan is offered to all eligible full-time and part-time employees. New employees have 31 days to enroll and benefits are effective the 1st of the following month. Employees pay a portion of the health plan contributions and co-payments.

Dental Plan

The Dental Plan is offered to all eligible full-time and part-time employees. New employees have 31 days to enroll and benefits are effective the 1st of the following month. Employees pay a portion of the dental plan contributions with a $50 deductible.

Vision Plan

The Vision Plan is offered to all eligible full-time and part-time employees. New employees have 31 days to enroll and benefits are effective the 1st of the following month. Employees pay a portion of the vision plan contributions. The Vision Plan covers 80% of eligible expenses, up to $500 in 24 months.

Prescription Plan

The prescription plan enables eligible employees to obtain prescription medications through local pharmacies for a one month supply or mail/internet for a three month supply.

Paid Leave

Paid leave is time off with pay for vacation, personal days, short-term sick leave, and holidays. Holidays include New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas Day, which are paid at time and one-half.

The accrual rate for eligible employees is based on their years of service and hours worked per pay period. A full-time (80 hours per pay period) employee accrues 7.1 hours of paid leave each pay period for the first four years of service. At the beginning of the 5th year an employee accrues 8.6 hours per pay period. And at the beginning of the 10th year an employee accrues 10.2 hours per pay period. Part-time benefited employees accrue paid leave on a prorated basis.

Disability Leave

Disability leave provides employees time off with pay during an illness lasting longer than a week. It accrues at the rate of 2.2 hours per pay period for employees who work 80 hours per pay period, and is prorated for part-time.

Cafeteria Plan

The cafeteria plan enables employees to put aside before-tax contributions to be used for eligible dependent care (maximum $5,000 per year) or out-of-pocket medical expenses (maximum $2,500 per year). Married employees who file separate returns are limited to $2,500 for dependent care.

403(b) Retirement Program

Adventist Health Simi Valley participates in the Adventist Health Retirement Plan (AHRP). Employees may begin contributing at any time. The plan offers both a before-tax and after-tax (Roth) option. Each employee can direct how his or her contributions will be invested in the more than two dozen funds available through the plan.

The hospital contributes 3% of your annual pay to the account of each eligible employee who works 1,000 or more hours in the calendar year. Additionally, the hospital matches 50 cents on the dollar of the employee's contributions up to a maximum of 4% of annual pay. Employees become fully vested after receiving hospital contributions for 5 years.

Life Insurance

The hospital provides $10,000 term life insurance and $10,000 accidental death and dismemberment (AD&D) for eligible full-time employees. Additionally, eligible employees may purchase voluntary term life and AD&D insurance at competitive rates.

Tuition Reimbursement

Tuition Reimbursement is offered to full or part time employees who have worked for the hospital for at least one year. The maximum reimbursement per approved program is $5,000 for full time employees and $2,500 for part time employees. Contact Human Resources for more information about this program.

Voluntary Benefits

Long Term Disability, Group Legal Services, Health Insurance for your Pets, Auto & Home Insurance, Critical Illness Insurance, Life & Accident Insurance or Identity Theft Protection are available through payroll deduction.

Professional Liability Insurance

The hospital pays for professional liability coverage for employees for work performed on AHSV premises within the employee's job description.

Jury Service

Employees summoned to jury service will receive up to 10 working days of regular wages (for 8- and 8.5/6-hour shift employees), eight working days (for 10-hour shift employees) or six working days (for 12-hour shift employees) in a calendar year, minus the amount the employee is paid for jury service by the summoning governmental agency.

Direct Deposit

Employees may elect to have their paycheck deposited directly into as many as 4 different accounts at financial institutions within the U.S.

Discounts

Employees can purchase discounted tickets to many attractions in Southern California.
