Associate Giving

As an associate at Adventist Health Simi Valley, we each have a unique opportunity to impact members of our community at their worst or best moments of life. When you make a gift to the Adventist Health Simi Valley Foundation, you share with others that your work is more than a job – it is a calling!

In 2019 the AHSVF is seeking to grow the percentage of associates that make a gift to the foundation. Our goal is to have 500 associates accept the invitation to give and consider becoming an Ambassador. An Ambassador is recognized because they make a monthly commitment to give back through reoccurring giving.

How do I become an Ambassador?

JLL, Sodexo, or Volunteer associates can make a monthly reoccurring commitment below by completing the giving form.

Adventist Health associates can give through payroll deduction by completing the payroll deduction form. To submit a request you can:

  1. Email it to
  2. Bring completed form to Foundation’s office, located at 2650 Jones Way, Unit #7
  3. Mail completed form to AHSV Foundation, 2975 N. Sycamore Drive, Simi Valley, CA 93065

What do I get?

As an associate, you are recognized annually in the Associate Giving Wall, located on the first floor of the Patient Care Tower. If you become an Ambassador, you are eligible to receive either a Badge Holder or Lanyard.

In 2019, you will also get a puzzle piece on which you can write your name and actively participate in building a culture of Love and Hope through the 2019 Associate iGive initiative along with a commemorative pin.

Gifts toward our goals must be received by December 31 for annual recognition.

How are my gifts used?

Your gifts directly benefit our community, no matter where you choose your gift to go. You can even consider making gifts to different areas of interest.

Our current efforts are toward securing the one-million-dollar goal to support the Advanced Care Expansion. These gifts will ensure a secondary Cath Lab for our hospital; bring electrophysiology, interventional radiology, interventional neurology, and peripheral vascular procedures to our community. These services are not currently available in Simi Valley and yet are needed by our growing community.

Many associates give to the HOPE fund. The HOPE fund allows us to Honor Our Proud Employees when they find themselves in tough situations of life. It’s a way to tell colleagues, “I believe in you and want to ensure support is available if you have an unexpected situation”.

You can also give to any of the other areas established by the AHSV Foundation like Healing Arts, Cancer Care, the Child Development Center and more.
