Street Nursing

In January 2019, Adventist Health and Rideout launched a Street Nursing Program to meet the medical needs of the growing population experiencing homelessness in our community. As part of its medical care delivery, the Street Nursing Program provides a tele-health platform which allows physicians in the Emergency Room to video conference with the patient in the field to determine the appropriate treatment. The Street Nurse team also works in collaboration with many community partners to assist with shelter and other community services they may need.

Leadership

The Street Nursing Team consists of a RN, Cindy Cooper, and a Patient Care Coordinator, Ashten Phillips. This team provides medical screenings and case management services to the homeless population two days a week at the local Coordinated Entry Sites.

Impact

Through community donations and grants the Street Nurse team is able to conduct outreach to the unsheltered population currently not enrolled in coordinated entry. In addition to medical care they provide hygiene supplies, dietary supplements, and encouragement to enroll in coordinated entry. To date the Street Nurse Program has outreached to over 1400 individuals and directly serving the medical needs of over 550 individuals, including over 1500 follow up visits.
