Information for Referring Providers

If you have patients who have bothersome tinnitus and would like some help, please use the referral form for our tinnitus management program to refer them to us. Please be sure to have the patient fill out the "Tinnitus and Hearing Survey" on the back of the referral form and fax this form, along with a copy of the patient's most recent audiogram, to us.

It is important patients have a hearing evaluation by an audiologist within the past two years. That evaluation will be reviewed at the time of the patient's individual tinnitus consultation appointment. We would be happy to perform an updated audiological assessment in our clinic if you prefer.
