Adventist Health Portland’s Resilience Center is an innovative and focused healthcare clinic for a select group of insurance plan members and Adventist Health employees. This multidisciplinary team provides patients with chronic health concerns with a broad range of care to support them in their home, the clinic, a care facility or in the hospital.

You will be surrounded with a higher level of care than is available in a typical primary care setting. Our goal is to manage the multiple chronic illnesses and other factors that impact your well-being and to help you reach the highest level of function and quality of life. The Resilience Center’s mission is to ensure that patients in the program receive the right care, in the right setting, at the right time.

We offer a comprehensive program designed to address chronic health issues such as:

Q: Who will be my primary care provider (PCP)?
A: Our team approach to care is part of what makes us successful. Both of our providers, Chad Lowe, DO and Diana Almy, FNP, will serve as your primary care providers.

A: Our team will provide a holistic approach to care. Patients find our care to be more complete and individualized than in previous experiences.

Q: Why was I selected for a referral to Resilience Center?
A: Patients are identified by a variety of methods including insurance plan, medication list, multiple chronic illnesses, and the potential for serious complications and decline.

Q: Why is it just me and not my spouse?
A: If you think your spouse qualifies for this level of care, please discuss it with their primary care provider.
