AHMC provides emergency, inpatient and outpatient services, and healthcare education to prevent, manage and treat chronic and acute conditions.

PRINCIPAL JOB DUTIES:

  • Responsible for review, evaluation and effective utilization of hospital services
  • Reviews all patient admissions for appropriateness, length of stay and certification
  • Performs concurrent reviews and advises medical, nursing, and ancillary staff of appropriateness of services rendered
  • Represents the Hospital’s and patient’s interests when interacting with health plans’ utilization review
  • Plans and coordinates continuum of care for patients by utilizing resources available in the community
  • Collaborates with the medical staff to support families and patients during hospitalization
  • Coordinates plan of care, including discharges with the patient, family, nursing staff and the physicians.
  • Arranges appropriate post hospitalization care to assure timely discharge of patients no longer needing acute hospitalization
  • Supports nursing department in the coordinated effort of discharge planning including medication reconciliation as necessary
  • Performs other duties as assigned

QUALIFICATIONS:

  • Graduate of an accredited school of Nursing
  • Current California RN
  • Minimum four years experience in a variety of nursing/social services settings
  • Working knowledge of concurrent review analysis including typical hospital medical procedures, Diagnostic Related Group and Prospective Payment System
  • Knowledge of social services and community resources
  • Ability to effectively communicate in written and oral form with patients, public, medical staff, and administrative staff
  • Ability to communicate in Spanish highly desirable
  • Capable of performing position related physical requirements
