Weight Management Programs

Adventist Health’s Engaged! Plan now covers the cost of certain weight management programs at 100 percent. (For Base Plan members, Full Plate Living is covered at 100 percent).

Weight management programs covered at 100 percent for Engaged! Plan members:

  • Weight Watchers
  • Full Plate Living: an eight-week online nutrition and weight management program available with a physician’s referral.
  • CHIP (Complete Health Improvement Program)
  • TakeTEN™: A proven ten-day lifestyle medicine program that uniquely combines expert medical care with nutrition, fitness, and spirituality into your personal lifestyle prescription. Covered by the Engaged! Plan with a physician's referral.

Weight management programs covered at 100 percent for Base Plan members:

  • Full Plate Living: an eight-week online nutrition and weight management program available to Base Plan members with a physician’s referral.

How does it work?

For reimbursement from Adventist Health, ask your doctor to sign the referral form before the program begins. Sign up for the programs, and attend the sessions. You must file for reimbursement, and must attend 80 percent of the sessions to be reimbursed. For Weight Watchers only, you will need to record your attendance on the attendance form.

Completed the program? Here’s how to file for reimbursement:

  • Ask for a complete certificate from the program.
  • Fill out the reimbursement form. Forms are available on the Connect intranet portal in the health plan folder.
  • Send the referral form, completion certificate (or attendance form) and receipts to: Joni.Brossmer@ah.org, or fax to: (916) 781-2441
