When patients are determined to be experiencing symptoms of a stroke, a collaborative team of individuals step into action to provide timely, seamless care. Care begins with EMS at the patients side. Once at the hospital the Emergency Department, combined with team members from X-Ray, laboratory, physicians, hospitalists, Physical therapy, critical nursing unit and then discharge planning.
