Home Care Services Ranked in the Top 25% of Region's Home Health Agencies for 4th Year in a Row

Awards & Recognition Adventist Health Home Care Services at San Joaquin Community Hospital (SJCH) was named to the 2009 HomeCare Elite list compiled by Outcome Concept Systems, Inc. (OCS), a Seattle based company providing data products and benchmark services for home health, hospice and private duty agencies. The listing of the top tier US home health agencies honors those that rank in the top 25 percent in their region based upon quality of care, quality improvement and financial performance.

“Our staff has consistently demonstrated their commitment to providing quality, top-level care that satisfies both our patients and the physicians we work with,” said Angie Almendares, Home Care Services director. “I’m so thankful to all the members of our staff for their hard work. This recognition is a true honor and speaks to the dedication and skill level of our caregivers and staff.”

OCS, which maintains the nation’s largest proprietary home care, hospice and private duty benchmark patient-level databases, determined the top home health agencies nation-wide through benchmarking based on quality measurement scores submitted to Home Health Compare, the Centers for Medicare and Medicaid (CMS), and the most current Cost Reports. Including the SJCH branch, seven Adventist Health hospital-based agencies spread throughout California and Oregon were named to the list.

An extension of SJCH, Home Care Services treats patients in the comfort of their own homes by providing basic medical procedures, medication education and condition-specific home safety as they seek to help prevent additional hospital visits.
