At Adventist Health, we acknowledge the role that spiritual, religious and emotional support has in contributing to health and healing. If you would like someone to pray with/for you or just offer some words of encouragement, our specially trained team of chaplains are here to help. Please fill out and submit the form below, and a member of the Spiritual Care team will personally visit you in our hospital.

Note: Requests using this online form will be available for response 8 a.m. to 5 p.m., Monday-Friday. If you want to meet with a chaplain outside those times/days, please notify your nurse or dial “0” on a hospital phone, and a chaplain will be contacted.
