  The LivingWell web site (link below) allows Adventist Health employees to take periodic wellness assessments, pursue a personal health improvement program, track health/fitness programs, maintain personal health records, obtain up-to-date information of health issues and medical conditions and explore the wide array of other online tools available on the web site to help you and your family achieve and maintain optimal wellness.

Disclaimer: Please note that the LivingWell website is a third party website and is not maintained by Adventist Health. Alere Health Improvement company ("Alere") maintains the LivingWell website. Adventist Health has entered into a contract with Alere that allows Alere to collect personal health information through the LivingWell website and restricts how Alere can use that personal health information. Disclosure of personal health information provided by you as a user of the LivingWell website is restricted to individuals and entities involved in your health plan and/or your healthcare and is protected in accordance with applicable laws. No other use or disclosure of your Personal Health Information will be permitted without your prior consent.

Please complete your biometric screening BEFORE completing this health risk assessment. Including your biometric scores (cholesterol, blood pressure, etc.) will provide you with more complete information about your health status.
